In collaboration with your approved course mentor, you will identify a specific evidence-based practice proposal topic for the capstone project. Consider the clinical environment in which you are currently working or have recently worked. The capstone project topic can be a clinical practice problem, an organizational issue, a quality improvement suggestion, a leadership initiative, or an educational need appropriate to your area of interest as well as your practice immersion (practicum) setting. Examples of the integration of community health, leadership, and an EBP can be found on the “Educational and Community-Based Programs” page of the Healthy People 2020 website.

Write a 500-750 word description of your proposed capstone project topic. Make sure to include the following:

  1. The problem, issue, suggestion, initiative, or educational need that will be the focus of the project
  2. The setting or context in which the problem, issue, suggestion, initiative, or educational need can be observed.
  3. A description providing a high level of detail regarding the problem, issue, suggestion, initiative, or educational need.
  4. Impact of the problem, issue, suggestion, initiative, or educational need on the work environment, the quality of care provided by staff, and patient outcomes.
  5. Significance of the problem, issue, suggestion, initiative, or educational need and its implications to nursing.
  6. A proposed solution to the identified project topic

Expert Solution Preview

Introduction:
The proposed capstone project topic is focused on addressing the problem of medication errors in the hospital setting. The increased occurrence of medication errors highlights the need for effective preventative measures. This project aims to provide a potential solution to mitigate medication errors in the hospital setting.

1. The problem, issue, suggestion, initiative, or educational need that will be the focus of the project:
The project will focus on reducing medication errors in the hospital setting. This issue is crucial as medication errors can lead to adverse events, prolonged hospital stays, increased healthcare costs, and even fatalities.

2. The setting or context in which the problem, issue, suggestion, initiative, or educational need can be observed:
The project will be observed in the hospital setting, primarily in the inpatient unit.

3. A description providing a high level of detail regarding the problem, issue, suggestion, initiative, or educational need:
Medication errors are a significant issue in healthcare, affecting patient safety and outcome. Although healthcare professionals are responsible for providing safe and effective medication management, errors can still occur, resulting in adverse events. Common causes of medication errors include miscommunication, lack of knowledge, workload, inadequate supervision, and inadequate training. Therefore, there is a need for effective strategies to prevent medication errors and promote patient safety.

4. Impact of the problem, issue, suggestion, initiative, or educational need on the work environment, the quality of care provided by staff, and patient outcomes:
Medication errors can significantly impact the work environment, as healthcare professionals may experience stress, anxiety, and potential legal issues when medication errors occur. The incidence of such errors may also impact patient outcomes, prolonging hospital stays, increasing healthcare costs, and leading to fatalities. By addressing the issue, healthcare facilities may benefit from improved employee satisfaction, patient safety, and cost-effectiveness.

5. Significance of the problem, issue, suggestion, initiative, or educational need and its implications to nursing:
The significance of addressing medication errors in the hospital setting has a major impact on the nursing profession. Nurses play a critical role in medication management, thus being proactive in mitigating medication errors can help nurses to avoid negative impacts on patient safety and quality of care. Through this project, nurses can be empowered with tools and strategies to improve medication safety and reduce medication errors, ultimately promoting a safer environment for patients.

6. A proposed solution to the identified project topic:
The proposed solution to mitigating medication errors involves using a barcode medication administration system (BCMA). This system involves scanning the patient, the medication container, and the medication label before administering medication to the patient. This system has shown significant promise in reducing medication errors and improving patient safety. By implementing BCMA, we aim to reduce the incidence of medication errors and improve patient safety in the hospital setting.
