Before beginning and planning Mrs. Gregg's care, I would want to find out about the following:

  • Physical set-up of her home such as where the bedroom is, is it a two-storey house, is the bathroom near her bed room, etc. to determine techniques into transfer, positioning and the like

What I would expect to obtain from the care plan are:

The action for the personal support worker is to include her in the plan of care to ensure involvement and as much as possible promote self independence at all times. Promoting independence is a crucial aspect of helping your patients manage their conditions and avoid setbacks. Learning these valuable skills requires a new way of thinking about health, but most patients will happily embrace self-management techniques.
